SMS Consent & Opt-In Information

Azov LLC operates Foreman (construction project finance at azovgroup.com) and the Sulamita Church Family Photo Booking System (at azovgroup.com/church-photos/). Both services use SMS/text messaging for transactional notifications only (no marketing messages). This page explains how consent is collected, what messages are sent, and how to opt out.
1. What Messages Foreman Sends
Foreman sends transactional SMS messages only — Azov LLC does not send marketing or promotional texts. Messages include:
- Account verification codes
- Two-factor authentication (2FA) codes
- Project status alerts and notifications
- Vendor compliance reminders
- Security alerts (e.g., new device login)
Message frequency varies based on account activity. Most users receive only a few messages per month.

1b. Sulamita Church Photo Booking SMS
Azov LLC also operates the Sulamita Church Family Photo Booking System at azovgroup.com/church-photos/. This service uses a separate toll-free number (+1-877-774-2792) to send transactional SMS messages only related to photo session bookings. No marketing messages are sent.
Messages include:
- Booking confirmation (date, time, arrival instructions)
- Session reminders (7 days and 1 day before the appointment)
- Waitlist notifications when a spot opens
- Rescheduling and cancellation confirmations
Messages are sent in English and Russian based on the family's language preference.
Message frequency is typically 2-4 messages per booking (confirmation + reminders). Message and data rates may apply.

"Your photo session at Sulamita Church is confirmed for May 17, 2026 at 8:00 AM. Please arrive by 7:50 AM. Questions? Reply HELP. Reply STOP to opt out."
"Reminder: Your Sulamita Church family photo is May 17, 2026 at 8:00 AM at the Church Gym. Please arrive 10 min early."
"Tomorrow! Your family photo at Sulamita Church is at 8:00 AM. Please arrive by 7:50 AM."
How Consent Is Collected for Church Photo SMS
When booking a family photo session at azovgroup.com/church-photos/, families may optionally provide a phone number. Below the phone number field, the following disclosure is displayed:
"By providing your phone number, you agree to receive booking confirmation and reminder texts from Sulamita Church (Azov LLC). Msg frequency varies. Msg & data rates may apply. Reply STOP to opt out. Reply HELP for help."
Providing a phone number is entirely optional — bookings work with email only. Consent is not a condition of booking a photo session.
To opt out, reply STOP to any message. For help, reply HELP or contact support@azovgroup.com.
2. How Phone Numbers Are Added & Consent Is Collected
Phone numbers are added to Foreman through the Account Settings page. The consent flow uses an explicit checkbox-based opt-in:
Step 1: The user (or an administrator on behalf of a team member) enters a phone number in the optional phone number field.
Step 2: The user must actively check an SMS consent checkbox to opt in to transactional SMS messages. This checkbox is unchecked by default and is never pre-selected or forced.
Step 3: The user must also check a separate Terms & Privacy checkbox to acknowledge acceptance of the Terms of Service and Privacy Policy. This checkbox is also unchecked by default.
Step 4: The user saves their profile. SMS messages will only be sent if the SMS consent checkbox was actively checked by the user.
Important:
- The phone number field is optional — users are not required to provide a phone number.
- Neither checkbox is required to submit the form or use Foreman. Consent is not a condition of purchase or use of the platform.
- If the SMS consent checkbox is not checked, no SMS messages will be sent even if a phone number is provided.
3. Consent Language
At the point of phone number collection, two separate checkboxes are presented. The exact text of each checkbox is shown below:
Checkbox 1 — SMS Consent (unchecked by default):
"By checking, you are allowing to receive transactional/informational SMS communications regarding account notifications, customer care, etc, from Foreman by Azov LLC. Messages frequency may vary. Data rates may apply, reply HELP for help or STOP to opt-out."
Checkbox 2 — Terms & Privacy (unchecked by default):
"By checking, I accept Terms of Service & Privacy Policy."
Actively checking the SMS consent checkbox constitutes express written consent under the Telephone Consumer Protection Act (TCPA) to receive transactional SMS messages from Foreman by Azov LLC at the phone number provided.
4. Visual Representation of the Consent Form
The following is a visual mockup of the consent form as it appears in Foreman Account Settings:

Both checkboxes are unchecked by default. Neither is required to use Foreman.
5. How to Opt Out
Users can stop receiving SMS messages at any time using either of these methods:
- Reply STOP: Reply STOP to any SMS message received from Foreman. A single confirmation message will be sent and no further texts will follow.
- Remove your phone number: Log in to Foreman, go to Account Settings, and remove or clear the phone number field. SMS notifications will stop immediately.
- Uncheck the SMS consent checkbox: In Account Settings, uncheck the SMS consent checkbox and save. SMS notifications will stop immediately.
After opting out, no further SMS messages will be sent from Foreman unless the user re-consents by checking the SMS consent checkbox again.

6. Data Privacy
Azov LLC does not sell, share, rent, or disclose mobile phone numbers or any information collected through SMS opt-in/opt-out to third parties or affiliates for marketing or promotional purposes. Phone numbers are shared only with Twilio, the third-party messaging provider used by Foreman, solely to deliver SMS messages.
